چکیده

For aged patients, the treatment of type A aortic dissection (TAAD) with the primary entry tear at the inner curve of the ascending aorta is a great challenge. Conventional surgical repair with cardiopulmonary bypass and deep hypothermic circulatory arrest is associated with high risk of mortality.[1] Thoracic endovascular aortic repair (TEVAR) with associated techniques to reconstruct all supra-aortic branches is still controversial.[2,3] Herein, we reported a new endovascular method.

Successful Repair of Type a Aortic Dissection in a Term Pregnancy: A Case Report

Stanford type A acute aortic dissection (AAD) is a very rare complication, with potentially lethal consequences in pregnancy. In fact, pregnancy has been regularly associated with the possibility of aortic. dissection in almost half of young women. Herein, we present the case of a 38-year-old woman in her 37th week of pregnancy. The patient’s medical history was indicative of G4L2Ab1(4 gestaiti...

Surgical repair and reconstruction of aortic arch in debakey type I aortic dissection: recent advances and single-center experience in the application of branched stent graft

BACKGROUND Aortic dissection (AD) represents a clinically uncommon aortic pathology which predicts a dismal prognosis if not promptly treated. In acute Debakey type I AD (ADIAD), aortic lesion extends from aortic root to even distal abdominal aorta among which aortic arch and its three main branches still remain a great surgical challenge for repair and reconstruction. Several decades have witn...

The Modified Chimney Technique With a Thoracic Aortic Stent Graft to Preserve the Blood Flow of the Left Common Carotid Artery for Treating Descending Thoracic Aortic Aneurysm and Dissection

While thoracic endovascular aortic repair is an effective treatment option for descending thoracic aorta pathology, it does have limitations. The main limitation is related to the anatomical difficulties when disease involves the aortic arch. A fenestrated, branched aortic stent graft and hybrid operation has been introduced to overcome this limitation, but it is a custom-made device and is tim...

In Situ Laser Fenestration Is a Feasible Method for Revascularization of Aortic Arch During Thoracic Endovascular Aortic Repair

BACKGROUND Reconstruction of the aortic major branches during thoracic endovascular aortic repair is complicated because of the complex anatomic configuration and variation of the aortic arch. In situ laser fenestration has shown great potential for the revascularization of aortic branches. This study aims to evaluate the feasibility, effectiveness, and safety of in situ laser fenestration on t...

A Great Start, but the Long-Term Durability Remains to be Demonstrated

Refer to the page 215-221 In this issue, Park et al. 1) present their experience with the use of a novel, fenestrated aortic arch stent graft (FASG) with a preloaded catheter for supra-aortic vessels. The authors have done an outstanding job highlighting a subject of increasing importance in acute clinical care, namely, performing thoracic endovascular aortic repair (TEVAR) in a short landing z...
